Position Summary

Reporting to the Program Manager, Medicine and Maternity, the Social Worker, in collaboration with the interdisciplinary team, provides comprehensive assessment, care planning and treatment coordination for designated clients and their families throughout the continuum of care. Working collaboratively with other health care providers and the multidisciplinary team, the Social Worker provides direct psychosocial care, education, navigation and advocacy to maintain or improve the health and social well-being and functioning of patients/clients/residents and their families in the Quesnel Health Services Delivery Area.

Shift Rotation/Hours of work: Days - 07:00 to 18:12, Rotating

GR Baker Memorial Hospital is a full care hospital with 27 Acute Care beds, 4 ICU beds and 5 crisis stabilization beds.

• Full Emergency Medical Care
• Laboratory
• Diagnostic Imaging
• Pharmacy
• General Surgery
• Internal Medicine
• Otolaryngology (ENT)
• Community Cancer Care
• Physiotherapy
• Respiratory Therapy
• Environmental Health
• Speech and Language Therapy
• Visiting specialists in Cardiology, Pediatrics, Orthopedics, Plastic Surgery, Urology and Ophthalmology
• Video conferencing for consultations with specialists or physicians in other areas.
• Home and Community Care and Home Support

Qualfications
• Bachelor's Degree in Social Work from an accredited university.
• Two (2) years recent related experience in a healthcare setting working with medically complex clients with concurrent disorders, or a combination of education, training and experience.
• Current registration with the British Columbia College of Social Workers.
• Current valid BC driver's license.

• Demonstrated ability to communicate effectively, both verbally and in writing.
• Demonstrated ability to provide service in a professional manner consistent with current social work practice standards.
• Demonstrated ability to engage in work with a variety of acutely behaviorally, psychologically, emotionally distressed patients and families.
• Demonstrated ability to counsel in grief situations.
• Demonstrated ability to solve problems, makes decisions, and takes action in a crisis management environment.
• Demonstrated ability to establish and maintain effective working relationships with patients, families, professional colleagues, and representatives of community agencies, and to advocate effectively on behalf of patients and families.
• Demonstrated ability to adapt and be flexible to changing demands and environments.
• Demonstrated ability to gather and work with statistical information.
• Physical ability to perform the duties of the position.
• Ability to operate related equipment such as Microsoft Office, Outlook, Internet, Excel.
