Job Description

POSITION SUMMARY
Reporting to the Director of Health & Wellness, the Health Manager oversees Health team operations and programming, including Community and Home Care, Chronic Disease and Diabetes and Adult-In Home Care. This role is an active community position which involves a high level of hands-on care, training and coaching, community outreach and building partnerships with clients and caregivers in the community. This position oversees department human resources related matters, training and development, program delivery and outcomes, financial revenues, and expenditures, and ensures that departments and management are in alignment with the Westbank First Nation government strategic goals.

EDUCATION/EXPERIENCE

  • Baccalaureate Degree in Nursing, Public Health, Health Promotion or an equivalent combination of education, training, and experience.
  • A minimum of five (5) years demonstrated experience in a Supervisory role with both people and fiscal responsibility.
  • Experience collaborating with Indigenous communities including demonstrated cultural competency and knowledge of Indigenous customs and traditional practices.
  • Experience in community work and collaborating with community partners in a collaborative way.
  • Experience leading complex teams with passion to mentor and develop Demonstrated experience leading a team of professionals with ability to support and mentor when necessary.
  • Solid knowledge of the principles, practices, and methods of program development, implementation, and evaluation.
  • Knowledge/experience with EMRs and other health programs such as Meditech, Panorama, Care Connect.

REQUIREMENTS

  • Current registration with the BCCNM required, maintain standards of conduct as per Nursing Standards and the WFN Client Care Policy.
  • Current BLS/First Aid training
  • Ability to properly use A1C machine.
  • Criminal Record Check with Vulnerable Sector Search with results acceptable to WFN
  • Valid Class 5 Driver’s License and Acceptable Driver’s Abstract
  • Maintains a high level of confidentiality and uses discretion on all matters relating to the affairs of the WFN.

JOB DUTIES/RESPONSIBILTIES

  • Direct, coordinates and monitors the delivery of all health services and daily functions of health and community staff providing access to clinical advice, support and resources including managing the budgets alongside people and programming.
  • Committed to supporting and implementing a syilx-based Indigenous Framework into Health and Community Home Care Programs and services while utilizing the WFN Community Plan and WFN Government Strategic Plan to continue to achieve the highest standard of program and services.
  • Oversee program operations including the establishment of strategic and operational work plans, program goals, & objectives within the Health Team.
  • Hire and train all Health team members, including training and orientation on all services, and supervise the day-to-day performance of the team.
  • Ensure exceptional interactions with the community and clients and provide ongoing coaching and performance management.
  • Provides required department statistics and reporting to Director, SLT and external partners such as Health Canada, Indigenous Services Canada (ISC), and other funding agencies to ensure financial accountability.
  • Liaises with community agencies and support groups and represents WFN at various meetings, events and activities where required with external partners. including Okanagan Nation (ONA), Interior Health, First Nation Health Authority (FNHA), etc.
  • Develops and amends policies and procedures associated with Health programs and services.
  • Committed to relationships and support of other WFN community programs and services and as the main Health resource.
  • Provide opportunities for education and support in community with a focus on chronic disease prevention and treatment, facilitated through family meetings, public presentations, and support group meetings.
  • Ensure community needs are met by assessing gaps and developing new programming and innovation to meet community needs (ex - comprehensive Chronic Disease Management program).
  • Identifies opportunities for ongoing and emerging opportunities for grant funding and contributes to writing applications in conjunction with funding coordinator.
  • Coordinates access and care for community members discharged from Kelowna General Hospital who are a part of the Health and Community Home Care Program or WFN members requiring home care services or support.
  • Participates in regional emergency programs/emergency operations center as required.
  • Acts as the Data Steward and provides leadership and understanding around Confidentiality, Privacy and Security Requirements for healthcare.
  • Maintains professional development by taking courses and participating in workshops.
  • Performs other duties and responsibilities as necessary in the performance of the position, and as assigned by the Director of Health & Wellness.

KNOWLEDGE/SKILLS/ABILITIES

  • Strong leadership and management experience.
  • Excellent ability to analyze problems, identify key factors and underlying causes and generate solutions.
  • Knowledge and skill in the areas of home care support, communicable disease control, and health promotion across the lifespan.
  • Proficient knowledge with computer software and other communications.
  • Knowledge of relevant legislation and First Nation Health Authority and Health Canada programs.
  • Commitment to policies, standards, procedures, and code of ethics.
  • Commitment to upholding values, culture, and standards of confidentiality.
  • Ability to adapt and practice in a culturally safe manner.
  • Have a flexible schedule with the availability to work evenings and weekends when required.
  • Proven ability to communicate effectively with staff, co-workers, and management both written and verbally.
  • Ability to prioritize and change direction as required.
  • Ability to plan, develop, implement, and evaluate programs and services.
  • Demonstrates stability, integrity, a supportive attitude, interpersonal skills, and a genuine interest in collaborating with people.
  • Leadership, interpersonal and relationship building skills.
  • Proficient knowledge in; Infant to Elder health, chronic diseases, General health, and disease prevention.
  • Possess cultural awareness and sensitivity.
